When Patients Become Family

St. James Parish Hospital’s 25-Bed Acute Care Department offers patients several levels of care each aimed at healing our patients through a combination of skill and compassionate, family-like care. In addition to traditional Acute Care, we provide Skilled Rehab/Swing Bed Services for patients requiring additional care to meet their goals for discharge.

In 2012, we also expanded with the opening of a Progressive Care Unit, which is equipped with advanced technology that allows nurses and physicians to more easily manage patients needing a higher level of care.

Our Acute Care Department is staffed with registered nurses and a support team that is highly skilled and focused on taking care of patient needs. In addition, we contract with a Nurse Practitioner and a Hospitalist—a specialist in the care of inpatients–who work together to help patients return home as healthy and quickly as possible.

Our Goal After Discharge

We work extremely hard to prepare patients and family members during transitions of care. Hospital Caregivers and the Case Management Staff team up during discharge planning to help assure patients and family members understand what is important in regard to follow-up care. It is our goal to adequately plan and educate to help patients avoid a return to the hospital.

  What can you do?

  • Ask questions about your condition and hospital stay
  • Ask questions about your medicines
  • Follow-through on recommended follow-up care
  • Find out how to get in touch with help after discharge if needed
