|  |  Your physician will make all preliminary arrangements for your admission to St. James Parish Hospital. It is important that the Registration staff receive complete insurance information before you enter the hospital.
If you have insurance, the following information will be needed:
- Medicare/ Medicare HMO Card
- Insurance Identification Card
- Medicaid Card
- Guarantee of payment from employer- if covered by Workmen’s Compensation
- Preadmission Certification, if required by your insurer.

WHAT TO BRING
Nightgown, pajamas, slippers, toothbrush, toothpaste, hygiene products, bathrobe.
WHAT TO LEAVE AT HOME
The hospital cannot be responsible for lost or stolen articles. Do not bring jewelry, money, or anything of value. | |
