This role is for the Registered Nurse who is licensed, professional, competent, and assumes responsibility and accountability for providing cardiac rehab therapy using a defined process in collaboration with the health care team to ensure optimal patient outcomes through exercise training, education, and counseling.  The nurse is responsible for developing and implementing individualized rehabilitation programs for patients with cardiac and pulmonary conditions.

Job Duties:

Patient Assessment:

  • Conduct comprehensive assessments of patients’ medical history, admission interview, physical condition, and exercise capacity while in pulmonary rehab according to established standards.
  • Evaluate risk factors and develop individualized rehabilitation plans.
  • Coordinates patient’s enrollment in the program

Program Development:

  • Design and implement customized exercise programs tailored to the specific needs and abilities of each patient.
  • Monitor and adjust exercise plans based on patient progress and feedback.

Education and Counseling:

  • Educate patients and their families about the benefits of pulmonary rehabilitation.
  • Provide guidance on lifestyle changes, nutrition, stress management, and smoking cessation.

Monitoring and Documentation:

  • Monitor patients during exercise sessions to ensure safety and proper technique.
  • Document patient progress, treatment plans, and outcomes in accordance with facility policies and regulatory requirements.
  • Monitor and adjust exercise plans based on patient progress and feedback.

Collaboration:

  • Collaborate with physicians, nurses, dietitians, therapist and other healthcare professionals to coordinate patient care.
  • Participate in multidisciplinary team meetings and case conferences.

Emergency Response:

  • Be prepared to respond to medical emergencies, providing immediate care and coordinating with emergency services as needed.

Patient Support:

  • Offer emotional support and encouragement to patients throughout their rehabilitation journey.
  • Aware of patient rights and protects the patient/family rights and confidentiality.
  • Address any concerns or questions patients may have regarding their treatment and progress.
  • Remains aware of patient rights and protects the patient/family rights and confidentiality.

 

Requirements:

Education:

Graduated from an accredited school of nursing
Registered Nurse License through the State Board of Louisiana

Certification:

Certification in Basic Life Support (BLS).
Certification in Advanced Cardiac Life Support (ACLS).

Preferred:

Experience in cardiac and/or pulmonary rehabilitation.
Experience in a clinical setting working with patients with chronic conditions.

Skills:

Strong knowledge of pulmonary rehabilitation principles and practices.
Excellent interpersonal and communication skills.
Proficient in using medical equipment and technology relevant to the position.

Shift:

Part Time; Monday, Wednesday and Friday; 8 hour shifts
